Kim has many years of experience of working locally for the community teams and supports Elm Hayes Surgery.  

Matt joined the PCN Team in August 2023, having previously worked in South Glos. Council's housing department and NHS Mental Health Services for the previous 10 years.  As a Social Prescriber Matt supports patients with social or non-clinical needs such as emotional/mental health support, housing, benefits and financial issues and social isolation.

Outside of work Matt is generally found playing the drums and guitar, cooking or camping.
